A Diagnostic Properties of FNAB in the Breast Lesion



Abstract
A clinical and statistical study was made in 598 cases with breast lesions in which fine needle aspiration biopsy was one at the Department of Surgery, College of Medicine, Hallym University, from January 1987 to December 1993. Among 598 cases,
excisional biopsy, incisional biopsy, and opration were odone in 373 cases, the manography for 324 cases, and ultrasonogrphy in 22 cases. The results of these preoperative three diagnostic procedures were compared with histologic diagnosis
postoperatively.
@ES The results of this study were as follows:
@EN 1) The clincial manifeststions were as follows; palpable mass, pain, nipple discharge, physical check up, and bleeding. The most common clinical manifestation was a palpable mass (415, 69.4%).
2) There were 10 male cases in which 9 cases were gynecomastia, and 1 case was carcinoma.
3) The most comon size of a breast mass ranged from 1 to 3 cm (50.8%). 170 cases (28.1%) were smaller than 1 cm. 82 cases (13.6%) were between 3 to 5 cm and 45 cases (7.5%) were larger than 5 cm.
4) Among 598 cases of FNAC exem, beingn breast lesions were found in 452 cases (75.6%), malignant were 44 cases (7.4%), suspicious were 9 cases (1.3%), and inadequate specimen were 89 cases (14.9%).
5) 373 cases were operated after FNAC and 334 cases were found to be benign breast lesions and 39 cases malignant.
6) The results of the diagnostic accuracy of FNAC was as follows; accuracy 93.6%, sensitivity 75.7%, specificity 96.6%, false positive rate 3.4%, false negative rate 24.3%, positive predictive value 92.3%, and negative predictive value 96.9%.
7) Mammography was performed in 149 cases and the results were as follows; accuracy 83.6%, sensitivity 85.7%, specificity 67.3%, false positive rate 32.8%, false negative rate 14.3%, positive predictive value 79.2%, negative predictive value
90.2%.
8) Ultrasonogram was performed in 22 cases, and among the 22 cases, histological diagnosis was reported in 10 cases, and the accuracy was 85.7%.
